[theme-reviewers] custom header

Note: this is a question that should be asked, in-ticket, and attempted to
be resolved with the reviewer directly, before escalating to the mail-list.
Just as reviewers should work with developers to process each Theme through
to approval; so also developers should work with reviewers to seek
clarification and answers to questions.
