[theme-reviewers] Support Contact form in theme options page.

Can a theme have a form for contacting theme author in theme options page?
Data like url, server os, desktop os, browser is collected by they have a
check box to not send that data.

I remember there was a discussion on something like this but don't remember
the conclusion or there wasn't any.
