[theme-reviewers] Theme ads widget/ code/custom contact form using template

The custom contact form is definitely Plugin territory. The custom Ad
widget is okay, though I would *recommend* leaving it to Plugins. The Theme
options to add header/footer scripts/CSS is fine (though these could also
be recommended to be left to Plugins).
