What is the source for the original code? If used in a Theme, the Theme
should provide the reference, including original work copyright notice and
license declaration.

Here is HTML5 Boilerplate: http://html5boilerplate.com/
According to the source docs( https://github.com/h5bp/html5-boilerplate ),
HTML5 Boilerplate is MIT-licensed

Here is Twitter Bootstrap: http://twitter.github.com/bootstrap/
According to the landing page and the source docs (
https://github.com/twitter/bootstrap ), Twitter Bootstrap is Apache
2.0-licensed.
(Side note: the Halflings subset of the Glyphicons icon set are also
released under Apache 2.0. See here:
https://github.com/twitter/bootstrap/issues/3942 )

But most importantly, *it is the responsibility of the Theme developer to
find and include this information in the Theme.* If they've not done so,
that is an absolute show-stopper for the review. Ask the developer to
re-submit with appropriate copyright/license information. (You're welcome
to pass along relevant links, but that's completely at your discretion.)
