[theme-reviewers] Theme Review Codex Page

Good morning, Theme Reviewers!

I've taken some time this morning to clean up the Theme
Review<http://codex.wordpress.org/Theme_Review>Codex page some more.
Hopefully it is more straight-forward, and has a
minimum of duplicate content.

Please let me know what you think, and fix (or let me know) if I've made any
mistakes, or added any incorrect information.
