Comment:

 I've duplicated the problem.

 The activity comments form should not show up on singular activity comment
 entries.

 eg.
 [[Image(http://i.imgur.com/DXoXErq.png)]]


 If you reply to one of these items, the problem will occur.

 I think this is a pretty big bug.  So we'll try and address this for 1.9.
