 Several tickets could refer to similar issues if this is about unattached
 media with the `/%postname%/` permalink setting:
 #13459, #15665, #18962, #24612, #31293, #34972, #48658

 I have had experience with the issue described in #34972, and I worked
 around it by deleting and replacing the image (#31293). @cbirdsong
 suggested a somewhat friendlier workaround:
 1. Go to the Edit Media page (click the title or "Edit" link in list view
 or "Edit more details" in grid view).
 2. Expand the Screen Options panel at the top.
 3. Check the box for Slug to show the Slug metabox.
 4. Edit the slug for the attachment and save changes.
 5. Edit the page's slug in the post editor (or Quick Edit).
