Comment:

 The alt text field in the image block is "local", which means you can have
 different alt text for different instances of the image throughout your
 website, depending on the context. It's on purpose that the alt text
 doesn't sync back to the media library in the way you describe it. That
 said, when you change the alt text "globally" in the media library, the
 next time you insert the image somewhere, it will use that alt text by
 default.

 There might be ways to improve that while still supporting all existing
 workflows. I suggest opening an issue at
 https://github.com/WordPress/gutenberg since this is a proposed change to
 the image block, which is developed there.
