 A report of this was filed in
 https://core.trac.wordpress.org/ticket/58960#comment:4 and @tgsrvrs shared
 a source image to reproduce this in the comments. We've closed it as a
 duplicate, but if anyone needs a source image that would be the place to
 grab one. :)
