 Since the purpose for including an image in a post probably will be the
 same when re-used, it should be stored with the attachment, as one or many
 (history). But user should ideally be prompted to select one, write a new
 or leave empty, with some simple explanation.

 When used as a featured image, may be we can figure out something smart?

 When used in other contexts, the theme/plugin should have option to
 suggest alt text for the specific usage.
