 I think the problem might have been that refreshing the page still has the
 orderby parameter, and the solution is to simply click on the Categories
 link in the main navigation(as it doesn't have any orderby parameters).

 I can confirm that when any sorting is applied, there are no hierarchy
 indicators, but I believe this is by design. I'm attaching screenshots
 without/with sorting.
