Comment:

 Sorry for the delayed review. WP doesn't generally do this sort of drop-in
 file support. It doesn't play nicely with variable scope, and it makes it
 difficult to return (as opposed to echo) the generated markup. You should
 be able to make all required customizations with the 'wp_list_categories'
 filter.
