 Yes, this is a change in TinyMCE 4.0. The 'code' and 'anchor' plugins can
 probably be added back and left unused if there are many users that enable
 them. On the other hand it would be pointless to add them if only few
 users are using them...

 It is also very easy to extend your plugin that enables these buttons and
 include the actual TinyMCE plugins. See
 http://codex.wordpress.org/TinyMCE_Custom_Buttons#Loading_a_TinyMCE_Plugin
 for more info. At this point I'm thinking this should be handled by a
 WordPress plugin.
