 Can you check the value of `$_SERVER['REQUEST_URI']` when visiting
 http://domain.com/wp-login.php? If it's anything other than /wp-login.php
 then this is a server configuration issue (or possibly a bug in
 wp_fix_server_vars(), but unlikely).
