 Discussed in IRC today ([https://irclogs.wordpress.org/chanlog.php?channel
 =wordpress-themes&day=2013-11-08&sort=asc#m75281 log]) and decided to go
 with option 2 above: "No fixed header when the menu is long and the header
 becomes more than 48px tall." Takashi to work on a patch.
