  * combined previous patches and diffed against trunk
  * incorporated get_the_modified_author function in meta box, revision
 page meta info and tooltips
  * did initial testing, created post before patch with several authors,
 off by one; applied patch, authors corrected (in display, on the fly);
 added new revisions, attribution correct, newest revision matches post

 ready for testing & review
