Comment:

 Discussion with Drew helps at least explain what is happening in the
 screen shot.  The crops are not based on "random" sizes, but they are also
 not based on the size of the image being represented.  It is just showing
 the Medium resized image at 100% zoom for high-aspect images (of any size)
 while showing the Medium size image at 50% zoom in most other cases.

 The result is that a 300x50 icon and a 900x150 photograph will both show
 up as narrow middle-crops of what should be a much larger picture.  The
 900x150 photo could easily fill the box, or could easily be resized to
 fit.  As the intention is to not do those things, I guess there's nothing
 else to add here.
