Comment:

 Patch posted for HTML mode. It is decidedly non-trivial.

 Known issues:
 * Icons will not appear when loading from HTML mode if the visual editor
 has never been loaded.
  * This is because the icon CSS is loaded when TinyMCE is first
 initialized. I'd propose shifting in some of the styles from ui.css into
 wp-admin.css (note that some already reside in colors.css), but did not
 include this in the patch.
 * The toolbar fading is acting funky.
  * I'll get to that next.
