 From Dashboard, click either the Quickpress Add Image/Add Media button.
 The resulting window has two Add Media "headers" at the top.

 I've reproduced this in Firefox 3.6.10, IE 8 and Safari 5.0.2, all on
 Windows XP, and with plugins deactivated and using the default 2010 theme.
