 After receiving a report of images being broken in 2.9, I found the Media
 Library's Insert Image Into Post button dysfunctional when using the Opera
 web browser.  With DEBUG mode, a PHP Notice appears, and the popup window
 fails to close/insert.

 Notice: Undefined variable: rel in /wp-admin/includes/media.php on line
 452

 In the upload and gallery tabs, no Notice is displayed, the window loads a
 WSOD, does not close, and does not insert the image.

 No other error information is reported by PHP.
