 Dremeda: Yes, it happens when you change the title of the media item to
 the same name as a page has.

 So:
 1. Media Item is uploaded, Title field == 'test'.
 2. A page is created after the media item is created. Page is named 'test'
 and permalink is example.org/test.
 3. The URL example.org/text goes to the Media Item created in step 1,
 rather than to the page created in step 2.

 I've been able to easily recreate this behavior on my test installs, but
 I'll go back through and check again.
