 I'm having this exact issue in WP 2.7.  Using FF and IE, still no luck...

 This just started happening, and I have not changed anything on my
 blog...no new plugins or anything of that nature.

 I use the insert media/image icon, it shows to be "crunching" but when
 it's finished, there is a blank white screen where the image used to be
 with what looks to be a small icon of a paper torn in half.  If I hit
 "insert into post" it inserts the same white image.

 Any ideas how to fix?
