 Not sure about the HTML entities fix. If there's a selection we do
 stripslashes(), aposfix(), html_entity_decode(), htmlspecialchars() and
 trim() on it, then we add some <p> and then do html_entity_decode() again
 (in effect reverting htmlspecialchars)?
