Comment:

 I tested this in [5012], though not on IIS with the given php.ini settings
 (I'm running on a host that uses some version of Linux/Apache, and I do
 not have control over PHP settings).

 The next/previous links I got were like this:

 http://bloglocation.wherever.com/index.php/page/2/

 (as they should be). I also verified by using View / Source that the
 entire path is being printed out in the link. Also, clicking on the links
 works fine.

 So, it seems as though this is some kind of IIS problem where it is trying
 to fix paths (whatever that means), not a WordPress bug. I haven't seen
 any documentation on the Codex about PHP settings for INI...
