Comment:

 This can happen when our systems are under heavy load, and has to do with
 delays due to MySQL replication across the servers. Usually it fixes
 itself after a short break, or only happens when we're having load issues.

 Not much we can do to fix it except keep doing overall improvements to the
 systems as a whole.
