Comment:

 A possible approach might be:

 Using the [http://www.appelsiini.net/projects/lazyload lazy load plugin]
 to lazy load all the gravatars. I think it takes into account not yet
 displayed images (hidden by "Toggle comments" link), we can trigger the
 plugin when you click the link.

 If you want I can try to make a proof of concept patch.
