They do. Rumors say some actually make a living out of it...But: Every
site is different, different people, concepts, contents, approaches in
presenting them. If you want to do your very own thing, it's very likely
that a ready-made theme would only be a foundation to build upon. Plus,
though some themes are highly configurable, there won't be a backend
switch for all its possibilities.

Honestly, you either need to find/ask/hire someone, or learn the basics
yourself:
