[theme-reviewers] Common mistakes by theme developers

Hello,

I have reviewed quite a few themes submitted to wordpress. When reviewing
themes, i see the developers of themes, keep repeating the same mistakes
again and again deviating from standard guidelines coding standards. So i
decided to put together these points in a single article, thinking that
educating the theme developers on these guidelines and thus makes it easy
for the review team.
